1. Baylor College of Medicine Children’s Foundation – Uganda, is a comprehensive HIV Care and Treatment Centre supporting the needs of HIV infected Children, Adolescents and their families intends to acquire the following services for two fi nancial years 2012/2013.

2. Baylor - Uganda now invites suitably qualifi ed and interested reputable fi rms to submit sealed proposals for the services mentioned below:

TERMS AND CONDITIONS TO BIDDERS

(a) All applications must be accompanied by the following (where applicable)

• Memorandum and Articles of Association.

• Certifi cate of incorporation or Registration

• Valid Trading License

• VAT registration where applicable

• Valid income tax clearance for 2011

• Reference letters from current/previous clients

• Evidence of current/previous contracts

(b) Proposal will be evaluated in accordance with the evaluation Criteria set out in the Bid document. Bidding is open to all local eligible bidders.

(c) Interested eligible bidders may obtain further information from Procurement Department at the address below

(d) The bid documents in English may be purchased by interested bidders upon submission of a written application to the address indicated below and upon payment of a non – refundable fee of 100,000/= (One Hundred Thousand Uganda Shillings Only) for each category. The method of payment will be cash payable to the Accounts Assistant at Baylor – Uganda.

(e) Bidders will be required to present the general Receipt as proof of payment before they can be issued with the bid document. The bid document must be accompanied by the original receipt.

(f) Bids in properly sealed envelopes marked “proposal for provision of …….. Services 2012/2013” should be addressed to the address given below.

(g) One original and two copies of completed bid document clearly labeled “Original” and “Copies” and sealed in one envelope must be hand delivered to the address give below.

(h) The proposal submission MUST be hand delivered at address given below by 4pm on 23rd February 2012.
