I develop itchy patches on my head
Dear Doctor,
I have itchy white patches on my head when my hair grows. It has been like this for a number of years. What is the problem?

Dear David,

Scalp problems can be embarrassing and uncomfortable. The most common cause of the problem is fungal infection of the scalp. Teania capitis is the most common fungi that causes problems on the scalp.

It is associated with lack of proper scalp hygiene and is more common during childhood.

Infections that occur during adulthood are usually secondary to a dry unhealthy scalp. Persons with a poor immunity are also at risk.

Drying of the scalp may be as a result of using very hard water, not drinking enough water and use of drying agents and toxins found in soaps. The white itchy patches indicate that you have a fungal infection on your scalp which needs to be treated using appropriate antifungal medication.

It is advisable to use oral antifungal medications in order to eliminate the fungus completely. The drugs should also be taken for at least three weeks. See a doctor for a prescription.

You also need to take better care of your scalp by taking in more fluids and avoiding soaps that can make the scalp too dry. Also eat lots of vegetables and fruits.

My anus itches when I walk a lot
Dear Doctor,
Since 2007, my anus itches whenever I walk a lot. This year, it has intensified as I sit for long. My anal opening gets wet up to the outside. The smelly fluid is pus-like but colourless. I have sought help in vain. I find it hard to walk because the anus gets too wet and itchy.

Dear Bob,
Anal itching can be a very uncomfortable symptom. The symptoms typically increase during hot weather or activity. This is due to increased irritation from the sweat.

Most commonly, itching is associated with poor anal hygiene. You may need to clean this area properly every after a bowel movement using wet wipes. Anal piles, fistulas, fissures (tears), and anal warts may also cause anal itching with an associated discharge.

Other causes include yeast infections and worm infestations. These can be treated. Irritating chemicals found in foods such as spices, pepper and hot sauces, may also cause these symptoms, so watch what you eat. The pus-like discharge indicates a bacterial infection and this is more common in individuals with fissures or tears in this area.

This can be treated with antibiotics. See a doctor to have a look at the discharge and examine the area to determine the actual cause. Do not be embarrassed.

In the mean time, try to keep clean.
