I get pain in the testis and within the muscle joining the testis and abdomen. It is on and off every three-four months. When it comes, I almost fail to walk.

One of the possibilities is twisting of the cord of the testis. Each testis is suspended on a cord made of tubes like a ball on a string. They include the blood vessels to and from the testis. The testis can twist and rotate the cord. This stops blood getting to the testis, and you will experience pain. In some boys and young men, this pain is very severe and an immediate operation is necessary. For some, it comes and goes like you say.

Another possibility is hernia. An hernia is a hole in the body wall of muscles. Through this hole, some intestines may protrude. It causes a swelling. It may be a painless swelling, coming and going for years. Sometimes, it may be painful. The pain and swelling may go down when you lie down. It may also be an infection in the tubes.
