Normally in adults, the foreskin pulls back from the head of the penis. In a few, this does not occur, but it does not stop one from ejaculating during sex.

This foreskin may cause a few problems. One may get recurring urine infections. For some, a tight foreskin causes pain on erection and during sex. Also it may fail to return after sex, trapping the head of the penis and causing pain and swelling. This problem can be solved by circumcision or splitting it. See a doctor. Failing to ejaculate during sex is different. It is uncommon. Typically, one can ejaculate when they masturbate, but fail when with a partner. The problem is mainly psychological. You fail to relax completely during sex. Physical causes include some drugs and spinal injuries.

You need counselling to understand why you fail to ejaculate. The counsellor will suggest ways to help you learn to relax and ejaculate inside your partner. Time, patience, understanding of the problem and an understanding partner are necessary.
