Dear Reagan,
YOU have got delayed or failed ejaculation (finishing). It is not very common. Typically, you can ejaculate when you masturbate, but not when having sex. This may sound good, but is frustrating to you and your partners.

Having children is also a problem. Causes are most commonly psychological.

The individual may have had a very strict upbringing, thinking of sex as ‘dirty’ or ‘bad’ and is unable to relax during sex. Differences in sexual orientation may be the cause and also some medicines.

Treatment is a long time process. You need to see a psychiatrist or sex therapist to work through the causes. Your partner should ideally be involved. You will be taught relaxation techniques to solve the problem over time.

If you have never ejaculated, or you do not ejaculate, then the problem may be physical.

Then it is best to see a specialist doctor (urologist) for an examination.
