Details of every item are obtainable from the office of the undersigned. 2. Applicants must submit the following:- (a) Name of the firm. (b) Name(s) of the director(s) and the powers of Attorney. (c) Name(s) of Banker(s). (d) Telephone, Postal and physical address. (e) Photocopies of certificates of registration or incorporation. (f) A copy of the Memorandum or Articles of Association. (g) Photocopy of VAT registration certificate. (h) Tax clearance certificate (original). (i) Original General Receipt for non-refundable tender fee. (j) Any other useful references e.g company profile. 3. Application forms are obtainable at the office of the undersigned on presentation of the General Receipt. 4. Applicants must pay a non-refundable tender fee in the following manner: (i) Markets, Parks, supplies and services 30,000/=. (ii) Construction /Rehabilitation - above 5m/= - 100,000/= and below 5m/= - 50,000/=. (iii) Security printing 50,000/=. 5. The tender fee above in (3) is payable in UCB Mubende on Account No. 34-02876-6. Present the pay slip to the District Cash Office, where a general receipt will be given to you in return. 6. Plain sealed envelopes clearly marked "Tender for ……" should be addressed to the undersigned and be received not later than 10:00 a.m on the 11th day of September, 2001. 7. Application forms are available at the office of the undersigned on presentation of the General Receipt. Bid documents will also be availed on presentation of the same. 8. The applicant and interested public are invited to attend the opening ceremony at the District Council Chambers in Kaweeri, Mubende. N.B: The District Tender Board is not bound to accept the lowest, highest or any bid. The closing date above does not alter that for the Nutrition Centres.
