Applicants must provide/attach the following:- (a) Evidence that the firm/individual has registered with the District for contract works in accordance with the notice from CAO's Office/Council decision. (b) Name of company/firm and its physical location and address. (c) Name of directors of firm/company and their contact address. (d) Name of bankers if any. (e) Evidence of VAT Registration. (f) Certificate of Registration/Incorporation. (g) Original copy of Income Tax Clearance 2001 and Trading Licence 2001. 3. All bidders must pay a non-refundable fee to the District Cashier as indicated below and attach original receipts to the application. Payment of non-refundable fee is as follows:- A. CONTRACTS/SERVICES Categories: Categories: 2, 3, 4, 5 at 100,000/= each. B. SUPPLIES Categories 13, 14, 16, 17, and 18 at 100,000/= each. The rest of the categories at 50,000/= each. 4. Applications in properly sealed plain envelopes and clearly marked "Tender for ……." Should reach the Secretary, District Tender Board, P.O. Box 61, Soroti by Thursday, 27/9/2001 at 12.00 noon. 5. All bids will be publicly opened on 27/9/2001 at 12.30 p.m in the Lukiiko Hall in the presence of bidders or their representatives.
