Licensing Requirements and Guidelines For 2014

1. General Requirements For all Applicants

1.1 Applicants should collect application forms from the following locations;

District Drug Inspector (DDI) based in the office of the DHO in every district Regional office located at the following locations;

 Central Region –Premier Complex, Nakawa.

 South Eastern Region –Plot 6 Rippon Gardens, Jinja

 Eastern Region –Plot No. 27, Kwapa Road, Tororo

 Northern Region – Erute road, Lira.

 Western Region:- Muganwa Centre, Plot30, Old Toro Road , Hoima

 South- Western Region: -Kamukuzi, Mbarara.

 West Nile Region – Plot 1 Mt. Wati Road, Anyaflo –Arua

NDA Headquarters, Plot No. 46-48, Lumumba Avenue, Kampala
