ALL drugs must be used cautiously and rationally during pregnancy, especially in the first trimester (first three months of pregnancy) when the foetus is still forming because they may cause developmental malformations.

Dear Doctor,
Last week, my daughter who is seven-months pregnant and HIV-positive, developed TB. She was put on ARVs including Efavirenz which I hear should not be used by pregnant women. We are worried she might produce an abnormal baby. Please advise.

Efavirenz is known to cause abornomalities in the development of monkey foetuses when used in the first trimester.

It can, however, be used in the second and third trimester if there are no other suitable alternatives and if one is sure to use contraceptives correctly and consistently after the pregnancy.
