I fail to ejaculate when having sex with my wife. My erection is good, but sperm do not come. What’s the cause? What can I do?

Dear Lawrence,

Ejaculation or the release of sperms, may be delayed or absent. This problem is not common. Sex can go on for a long time, but the man and his partner become frustrated.

Have you ever ejaculated? If you have never, you may have a physical problem with the tubes leading sperms out. See a doctor, or urologist.

There are some drugs which can delay ejaculation. Other physical causes include diabetes, old age and spinal cord injuries.

Do you sometimes ejaculate, for example, while masturbating or with another partner? The most common causes are psychological. The man cannot relax and ‘let go’. He may feel the vagina is ‘dirty’, or cannot relax with his partner, or thinks sex is bad. Treatment includes counselling and sex therapy. Home exercises recommended include starting by masturbating without your partner and then after some time, with and helped by your partner, before penetration.

The problem can be dealt with, but it may take some time and patience.
